The “FWD Health & Go” Membership Program (the “Program”) is provided and managed by FWD Life Insurance Company (Bermuda) Limited (incorporated in Bermuda with limited liability) (“FWD Life”). This Program is a membership program but not an insurance plan. |

Free FWD Health & Go 12-month plan includes one main member and 3 complimentary affiliated members (applicable to spouse, parents, children aged below 18 of the main member, or domestic helper) (“The Offer”) |

The promotion period is valid from 4 January 2024 to 31 December 2024 (both dates inclusive) (“Promotion Period”). |

Designated insurance products include the basic plans of the following individual insurance products that are underwritten by FWD Life. The respective Designated Insurance Products are subject to change as determined by FWD Life from time to time without further notice. The policies of Designated Insurance Plans are subject to the respective policies’ terms and conditions. Please refer to the product brochures and policy provision for details:

— MyCover Critical Illness Plan
— MyMillion Medical Plan
— vPrime Medical Plan (VHIS Plan Certification Number: F00045) (VHIS provider registration number: 00036) This service is optional and does not form part of the Terms and Benefits of vPrime Medical Plan. You have the right to opt-out this service. Please inform FWD in writing if you do not want to receive this free additional service.

The Offer is only applicable if the following requirements are satisfied:
— Applicant(s) must submit application of Designated Insurance Product(s) underwritten by FWD Life through FWD online platform with specific promo code during Promotion Period; and
— The application of such Designated Insurance Product(s) must be accepted by FWD Life and issued on or before 31 Janurary 2025 |

Eligible Customer(s) must be the Policyholder(s) of the Designated Insurance Product(s) and maintain the policy(ies) of such Designated Insurance Product(s) in force during the usage period of the Offer. Otherwise, FWD Life reserves the rights to cancel his/her eligibility of the Offer without notice. |

Each Eligible Customer(s) is entitled to the Offer once only during the Promotion Period. |

8. |
Eligible Customer(s) must be aged 18-100 and a permanent resident of Hong Kong and required to successfully registered as main member(s) of FWD Health & Go to enjoy the Offer. For Eligible Customer(s) who is/are existing main member(s) of FWD Health & Go prior the Promotion Period, his/her membership of FWD Wellness Pass will be extended for 12 months through The Offer. The Offer is not applicable to existing affiliated member(s) of FWD Health & Go. |

Unless specified, the Offer cannot be used with other promotions and/or offers. |

10. |
FWD Financial Limited (“FWD Financial”) will issue a redemption letter within 3 months after the issuance of the Designated Insurance Product policy (“Redemption Letter”) to Eligible Customers’ email address registered at application. |

11. |
FWD Life and/or FWD Financial will not re-send or re-issue the Redemption Letter if the promotion code has been used or Eligible Customers fail to use and/or redeem the Offer within the specified period as stated in the Redemption Letter. Redemption Letter will not be re-issued in any circumstances and the Eligible Customers bear the sole responsibility to maintain a proper custody of the Redemption Letter and/or the promotion code. |

For any products and/or services given under the Offer and the use of the products and/or services will be subject to the terms and conditions imposed by the supplier. FWD Financial is not the supplier of products and/or services. FWD Financial will not be responsible for the availability or quality of the products and/or services or the consequences of using the products and/or services (including but not limited to the service quality of the providers) under any circumstances. |

The application submission date and the issuance date of policy of the Designated Insurance Products are based on the records of FWD Life. FWD Life and/or FWD Financial will not be responsible for any delay, loss, error or unrecognized situation due to computer and/or Internet’s connection, technical problem, malfunction or accident. |

15. |
The Offer cannot be refunded, changed or redeemed for cash, exchanged for other gifts or services. |

16. |
If the policyholder cancels or surrenders the policy of the Designated Insurance Product(s) before the end of the first policy year, FWD Life and/or FWD Financial shall have the right to recover the fee of the Offer. |

17. |
FWD Life and/or FWD Financial reserves its absolution discretion to vary this Offer without further notice. |

18. |
In case of any dispute arise in the contents of this leaflet or this Offer, FWD Life and/or FWD Financial has final interpretation right on the contents of this leaflet. For any dispute, the decision of FWD Life and/or FWD Financial shall be final. |
